FAQ - Form SF-86

What is the purpose of Form SF-86?
If you were to go on eBay right now you would find hundreds upon hundreds of listings for jobs that are in the process of being filled. How many of those jobs would involve running any type of background check? I can do this myself! If I don't know the job details then I don't care how well the company is vetted. After all, why bother having a background check on the job openings in the first place? Well, in my experience, they aren't being vetted for the reason that they should be. The purpose of a background check is to ensure that a prospective applicant is who they say they are. For some people, that is a good thing but in the case of eBay, I highly doubt that the vast majority of their applicants will have a criminal record. For the rest of the positions, I have a far higher standard for hiring. The purpose of a background check is to ensure that a prospective applicant is the right candidate. But if they can't pass that check then why would I go forward with them in the first place? Let's assume that I was hiring someone to work for me; what is the first check I would do? If I have a job opening and the only thing I'm concerned about is the candidate's criminal history then why not go for a full review or criminal history record check. I could do that myself and I would know immediately if the candidate was a liar and if they were lying about a crime that they have committed or a conviction. I would also know immediately whether we had shared a home address at some point in the past and that is a felony conviction that would disqualify someone. Furthermore, I can get it done myself and be sure of myself, but that isn't what eBay needs. If I wasn't hiring someone, so why would I do that, and why is it required that I should do it? What good is a background check if I don't vet the person on the first go? The bottom line is that a background check is not meant to be an end-all-be-all because a criminal record will eventually show itself. But what it can easily become is a way to check off all the boxes you need done to get the job done. This is called “over-screening” and it can lead to people being selected that should never have been selected in the first place.
Who should complete Form SF-86?
Filling out an F-O-C-K (Federal Employee Questionnaire) or Federal Taxpayer Identification Number (TIN) for the first time is a legal necessity. The F-O-C-K is needed to determine your eligibility, including questions about financial information, foreign affiliations and political positions regarding your service in the U.S. military. If you've had two prior attempts to receive a Form SF-86, you will automatically be asked to fill out this additional form. Questions you should ask yourself before completing an F-O-C-K 1) What is my income range? What has changed? How much do I earn from a variety of sources? How much is available to me from a specific source? 2) How is my income derived? Do you have income from a specific career/job area? 3) Have I filed a tax return for last two years? What happens if I had not? 4) Do I have a business? Why is my business relevant for employment consideration? 5) Am I married? If so, have I filed a tax return and if so, can you answer the question, “Are my spouse and/or defendants included in gross income?” 6) Do I consider myself a government employee (USSS, U.S. Coast Guard, etc.) 7) Are there family members (spouse, children, parents, etc.) with whom I work? What if my family member is in the military? 8) Which position, if any, do I hold? Please list the position, and provide a description of the work that you are seeking work.

The United States Government and the Passport Division will only give you specific instructions about submitting, at your own expense, your personal financial information via mail sent from the U.S. Embassy or Consulate in your destination country. If your travel documents include an approved document or passport photo that displays your personal identifying information, you should submit a photocopy of your passport or your passport photo with it if you intend to carry it with you. If you are traveling on an accompanying visa issued by a foreign state, you should submit additional forms of personal financial information, such as your bank account number, credit card number, and social security number, and make them available for review at the Embassy or Consulate. For travel documents containing a name, address, or phone number, such as a passport, your home address or business address, and a credit card number or social security number, the travel document may be accompanied by a photocopy of your passport and a copy of your accompanying visa. The SAP is available for review at either the Embassy's office, at the visa counters or through the Consulate's U.S. Consular Officer or a U.S. passport office if they do not have a physical location where you can review the SAP.
